E101 Camera Policy w/Acknowledgement: Many companies specifically prohibit the use of cameras in the workplace. E102 Cell Phone/Texting Policy w/Acknowledgement: For companies that have a policy regarding the use of cell phones (with or without texting or camera capabilities) while performing work duties. E103 Computers, Electronic Mail and Voice Mail Usage Policy: Gives your company the right to regularly monitor telephone calls, voice mail or electronic mail messages including blogging. The employee or consultant is notified that your company has the right to inspect the contents of computers, voice mail or electronic mail in the course of an investigation triggered by indications of unacceptable behavior or as necessary to locate needed information that is not more readily available by some other less intrusive means. E104 Daily Time Sheet Form: This form helps you keep track of each employee's hours on a daily basis. This form can be used in conjunction with the Employee Attendance Record.
E105 Dress Code Policy w/Acknowledgement: All types of businesses should implement a dress code policy to establish standards of dress. E107 Employee Attendance Record Form: Use this form to record the attendance of an employee on a monthly basis (or a different interval). This form gives a detailed record of the reasons for the employee's absences.
E108 Employee Emergency Information: This form captures an employee’s emergency contact information in addition to information regarding health concerns, allergies, and other limitations that would otherwise not be permissible on an employment application. E113 Employee Self Evaluation Form: Give this form to an employee before a performance review or at any time to evaluate an employee's understanding of their job, your organization, its structure and the employee's role in it.
E114 Employee Verbal Warning Form: Managers and supervisors use this form to document a “verbal warning” given to an employee. E115 Employee Written Statement of Warning Form: Managers and supervisors use this form to document a “written warning” given to an employee. E116 Employment Application Package: This package contains all of the documents a prospective employee will complete and submit in order to be considered for employment. E117 Employment Offer/Deal Memo: Use this document when you have selected the candidate you wish to hire. E118 Entry Code, Employee Badges, Alarm Codes and Office/Shop Keys Policy: This form is provided to and signed by the employee acknowledging the employee’s understanding and acceptance of your company policies regarding the use (and sharing of) entry and alarm codes, the use (and return of) employee badges and the use (and return of) keys to your facilities. The form outlines the procedures an employee must follow if entry or alarm codes are compromised (discovered by anyone not authorized to have those codes) and what an employee is to do if an employee badge or keys are lost or stolen. E119 Job Analysis: This form aids you in the employee selection process. Use it with the Job Description form to create recruitment materials, such as classified ads, to determine what attributes you are seeking in an employee.
E120 Job Description Form: This document is used to provide a new employee (or an employee switching positions within your company) with detailed information about their job (hours, work conditions, general duties, etc.) and how their job relates to other positions in the company.

E122 Non-Disclosure Agreement Form – Employer Version: A non-disclosure agreement (NDA), also known as a confidentiality agreement, confidential disclosure agreement (CDA), proprietary information agreement (PIA), or secrecy agreement, is a legal contract between your company and your employees that outlines confidential materials or knowledge you may wish to share with your employee for certain purposes, but wish to restrict access to. It is a contract through which your employee agrees not to disclose information covered by the agreement. The NDA creates a confidential relationship between the parties to protect any type of confidential and proprietary information or a trade secret. E123 Non-Compete Agreement Form – Employer Version: A non-compete agreement is legal contract in which an employee agrees to not pursue a similar profession or trade in competition against your company. A non-compete agreement is bound by traditional contract requirements including the consideration doctrine. The use of such clauses is premised on the possibility that upon their termination or resignation, an employee might begin working for a competitor or start a business, and gain a competitive advantage by abusing confidential information about their former employer's operations or trade secrets, or sensitive information such as customer/client lists, business practices, upcoming products and marketing plans.

E125 Personnel Change Notice: Use this form to record changes in an employee's status--a new hire, a terminated employee or an employee who has changed position or salary.
E126 Request for Payroll Correction: This form is provided to an employee if the employee believes there is a problem with the number of hours or rate of pay from a previous paycheck. The form greatly simplifies the process and allows the company owner/manager to correct the problem when the form is submitted rather than be forced to interrupt the workday to immediately resolve the issue. E127 Video Surveillance Policy: Do you have cameras in your office? If so, you need a written policy and signed statement from each employee. This form gives you right at any time to employ a video surveillance system to electronically record the business activities of the company and its employees. It states that there is no legitimate expectation of privacy in your company’s workplace and federal and state laws do not preclude or prevent the company from electronically observing and recording activities within the public areas of its offices (never in restrooms, changing room, patient examination or shower rooms). It defines the penalties for a violation of this policy.
